What does "Medical Home" mean?
"Medical Home" describes an approach to health care that emphasizes the partnership between health care practitioners and families, providing care for Children and Youth with Special Health Care Needs (CYSHCN) that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective.

What is the PA Medical Home Initiative?
The PA Medical Home Initiative (PA MHI) is a non-profit program funded by the PA DOH and housed at the PA Chapter of the American Academy of Pediatrics. The PA MHI works with pediatric and family practices across the state, helping them to increase their "medical homeness".

So what is this network for?
Parents have repeatedly told us that the most valuable resource for improving the lives of their children has been advice from other parents who have "walked the walk". These Parent Partners requested an online forum in which they could learn from others, share their own knowledge, and offer support.
